If you would like to submit an FOI Request or an FOI Complaint (on a previous request) using the Manual Forms provided above, kindly download the relevant form and print it, fill in all details, sign and return to the FOI Officer of the Public Authority to whom you are making the request/complaint. You can submit these forms by email or by post. Alternatively, you can download and directly fill in the Electronic Form required and send it by email or by post.
IMPORTANT! Remember to attach a scanned copy or a photocopy of your ID card/Residence Permit whenever you submit a request using either the Manual Forms or the Electronic Forms.

Kindly mark your submissions with "FOI Request - Attn. FOI Officer" or "FOI Complaint - Attn. FOI Officer"(as applicable), in order to make sure that the request/complaint is properly channelled.
Requests/ Complaints received over the weekend or on public holidays will be addressed on the next working day. All references to "working days" in relation to the FOI Act should be understood as "Monday to Friday".
